CCCADA Award Nomination Form

This is a nomination for: *
  1. Below are the procedures for the nomination of candidates for the CCCADA Service and Honor Awards. It is important to meet each requirement. Nominations shall be judged from the information provided on this nomination form.
  2. Criteria for Service Award:
    1. The candidate must have a strong recommendation from his/her community college.
    2. The candidate must have served as a California Community College Athletic Administrator for 10 or more years.
    3. The candidate must have been an active and contributing member of the CCCADA.
    4. A minimum of two (2) years shall have elapsed since the completion of service as an Athletic Administrator.
  3. Criteria for Honor Award:
    1. Participation as a member of athletic administrative bodies such as the Commission on Athletics, Executive Board of CCCADA, Coaches Association Executive Board, or other athletic leadership activities.
    2. Sponsorship of state level activities such as sport championships, COA activities, or other.
    3. An active involvement in his/her community and its relationship with community college athletics.
    4. The candidate must have a strong recommendation from a California Community College Athletic Director.
    5. A minimum of two (2) years shall have elapsed since completion of the most recent service upon which the award is based.
  4. Procedures for Selection: All nominees shall include the following:
    1. Nomination form.
    2. Biographical sketch.
    3. Recommendation of the nominator and/or college.
    4. Two letters of recommendation which must describe the accomplishments of the nominee.
  5. Schedule :
    1. December 1 - Call for nominations sent to all CCCADA members.
    2. March 15 - All complete forms submitted to CCCADA president.
    3. June - Awards announced at annual convention.
